We are a hybrid practice offering access to care through several avenues.

Fee-for-Service/Insurance Billing:
We are in-network with Cigna, FL Medicare, FL Medicaid (Better Health), and Tricare.
We submit claims and bill the plans listed above for visits/care provided. Cost of care depends on your plan benefits and other factors like complexity and time.

DPC Monthly Subscription:
$75/month, which covers all clinic visits including your initial intake, an annual wellness exam, & problem/sick visits. Services include in-person appointments, telemedicine appointments & unlimited messaging with your provider.

Please note there is a one-time $100 enrollment fee (waived for previously established patients).

Cash Pay:/Pay Per Appointment:
- Initial Visit: $180
- Follow-up visits: $120
